Primary Care
Transitions Clinic is a medical home for individuals and their families with a history of incarceration. The clinic provides primary care health services to the post-release prison population, which includes:
- Management of chronic medical conditions
- Refills of essential medications
- Substance abuse treatment and counseling, including suboxone.
We also refer patients to:
- Laboratory and radiology services
- Psychiatry and mental health services
- Specialty care
