Transitional and primary care services tailored to individuals returning from prison and their families. Case management and patient navigation by previously incarcerated community health workers. Substance abuse treatment, urgent care, laboratory, psychiatry, social work, and mental health services. Some sites include pharmacologic treatment for alcohol, cocaine and opioid addiction. Other sites provide dental, podiatry and optometry.
We aim to reach all patients with chronic medical conditions recently released from prison.
