Post Release Wellness Project
The Post Release Wellness Project (PRWP) is a unique partnership between three organizations:
- Transitions Clinic
- Legal Services for Prisoners with Children (LSPC) - a non-profit advocacy group
- City College of San Francisco’s (CCSF) Health Education Department – the home of the nation’s first community health worker (CHW) certificate program
PRWP promotes wellness for San Francisco’s formerly incarcerated population through:
- Culturally competent primary care health services and case management for formerly incarcerated individuals
- Advocacy for policies that support equal access to quality health services on the state and local level
- Training for front-line providers including community health workers
Post-Prison Health Worker Certificate
PRWP led to the nation’s first college-based vocational education certificate to train front-line public health providers in working with formerly incarcerated individuals. This one year certificate program is open to all students at CCSF and provides a core curriculum on the care of this population.

- Chronic Conditions
- Working with Formerly Incarcerated People
Training Site
The Transitions Clinic is a primary training site for the Post-Prison Health Worker Certificate Program. Through one semester rotations, CCSF CHW Trainees work along-side Transitions Clinic Community Health Worker’s to gain on-the-job experience in direct service provision to this population. Transitions Clinic CHW’s serve as preceptors for these trainees.
Transitions Clinic Replication:
PRWP provide consultation and training for other organizations interested in setting up a Transitions-like clinic in their community. Consultation and training services include:
- Needs assessment
- Project design and management
- Clinical services
- Community health worker training
- Team development
- Community building
